The Home and Community Care (HACC) Program is a joint Commonwealth-State cost-shared program which aims to provide home and community care services to the Program's target population. That population is the frail aged or younger disabled people with moderate or severe disabilities who are at risk of premature or

inappropriate admission to long term residential care (ie. care in a nursing home or other institution) and the carers of those persons.
