(2) What is the name and address of each centre.
(3) What charges are applied by each centre to the parents of children enrolled at the centre.
(4) What proportion of funding under each category has been provided to (a) the parents and children at each centre and (b) each centre for each year the centre has been open.

(b) Information on the total amount of Childcare Assistance paid to community based and private centres for each year they have been open is unable to be provided.
